A 28-year-old woman, gravida 2 para 1, at 36 weeks of gestation, presents with a 3-day history of malaise, nausea, vomiting, and right upper quadrant abdominal pain. On examination, she is jaundiced and has mild hepatic encephalopathy (WH 1). Laboratory investigations reveal elevated liver enzymes, hypoglycaemia, coagulopathy, and elevated serum ammonia levels. An abdominal ultrasound shows no signs of gallstones nor any dilation of the biliary system.
