A 67-year-old man with alcohol induced cirrhosis has been abstinent from alcohol for four years. He feels well, but recently his albumin has fallen to 32 g/dl and his bilirubin risen to 37. He has diuretic controlled ascites. A surveillance abdominal ultrasound scan identifies a new lesion in the right lobe. His AFP is 100. He has a contrast enhanced MRI scan which identifies three lesions in his liver, 3 cm in segment 6, 1.8 cm in segment 4 and a further 2 cm lesion in segment 6. The characteristics are as shown below:
